Permission to Practice Info


Permission to Practice – all student-athletes must have permission to practice granted by the Activities Office prior to practicing with any Jefferson athletic team. To be given permission to practice, please visit the Jefferson Activities Registration website at: and complete a registration form for all student-athletes in your family. We only accepting online registrations. You must have a copy of your most recent physical to complete the process. If you need assistance obtaining a copy of a completed physical, contact your doctor’s office or the activities office.
To be eligible to practice with a Jefferson athletic team, these 2 items must be completed and/or submitted through the Jefferson Activities Registration website.
  • Physical – to participate in school sponsored athletic activities, a student-athlete must have a current physical on file with the school. This physical must have been completed within the 12 calendar months. A 30-day grace period is allowed for insurance coverage purposes. If an athlete’s physical expires they will be unable to practice or compete until a new physical in on file.   • Concussion form –
    For the 2020-2021 athletic year we are utilizing a new concussion baseline testing program.  Jefferson is asking all student athletes to complete this at home with the use of their Chromebook or another computer that has a keyboard.  Please see the attachment with the instructions on how to complete this.  Feel free to contact Suzi, our athletic trainer, at with any questions or concerns!  If you played baseball or softball, you DO NOT have to take this again."
